CERTIFIED NURSING ASSISTANT (CNA)

BASIC FUNCTION

To provide routine daily nursing care and services that support the care delivered to patients/residents requiring long-term or rehabilitative care, in accordance with the established nursing care procedures and as directed by your supervisor.

CHARACTERISTIC DUTIES AND RESPONSIBILITIES

ESSENTIAL FUNCTIONS

1. Provides care as directed by the professional nurse to patients/residents requiring long-term, rehabilitative care or restorative care.

2. Reports observations and other pertinent information related to the care of the

patient/resident.

3. Documents objective information related to patient/resident care.

4. Provides services that support the care delivered to the patient/resident.

5. Provides input in the formulation and evaluation of standards of care.

6. Participates in the development of unit objectives.

7. Participates in the identification of staff educational needs.

8. Conducts self *in a professional manner in compliance with unit and facility policies.

9. Participates in the quality assessment and improvement process, and activities.

10. Performs other related duties as assigned or requested.

EXPOSURE RISK

The Certified Nursing Assistant (CNA) is at high risk for exposure to blood and body fluids.

SUPERVISION RECEIVED

Receives administrative supervision from the Director of Nursing, the Assistant Director of Nursing or the Nursing Supervisor. May receive functional supervision from registered or practical nurses working on the unit.

SUPERVISION EXERCISED

Exercises functional supervision, responsibility, and accountability necessary for carrying out assigned duties.

MINIMUM QUALIFICATIONS

1. Graduation from high school.

2. Previous Nursing Assistant experience or student nurse preferred.

3. Completion of certification program for Nursing Assistants preferred.

MINIMUM PERFORMANCE STANDARDS:

GENERAL PATIENTIRESIDENT CARE

1. Patients/residents are identified by the use of the identification band prior to administering treatments, serving meals~\etc., as necessary.

2. Patient/resident is approached in a kind, gentle and friendly manner. Respect for the patient's/resident's dignity and privacy is consistently provided. Confidentiality of all patient/resident 'information is maintained.

3. All grievances and complaints made by the patient/resident are reported to the Charge/Staff Nurse.

4. Assistance is provided with admissions/transfers/( discharges.

a. Room is stripped after patient/resident transferred/discharged.

b. Patients/residents are moved to accommodate unit needs.

5. Specimens are transported to the appropriate areas.

6. Patient/resident rooms are restocked.

a. Rooms are neat and orderly.

b. Excess supplies and linen are removed.

7. Care plans are reviewed daily to determine if changes *in the patient's/resident's daily care routine has been made.

8. Assistance with patient/resident care is provided as directed.

a. Baths are given.

b. Linens are changed.

c. Patients/residents are properly positioned.

d. AM and PM care is given.

e. Vital signs are taken.

9. Assistance with restorative programs as

a. Positioning and alignment e. Range of Motion

b. Skin integrity f. Activities of Daily Living

c. Bowel and bladder g. Dining

d. Ambulation h. Emotional

10. Hallways are cleared of equipment or on one side.

11. The unit is maintained in an orderly fashion.

12. Meal trays are distributed and picked up.

13. Patients/residents are assisted or fed at meal times as directed.

14. Bed pans, urinals and emesis basins are distributed and removed as requested.

15. Routine specimens are collected as directed.

16. Assistance with treatments is provided as directed.

17. Patient/resident call lights are promptly answered. Appropriate responses to requests are provided.

MAINTENANCE OF SUPPLIES/Equipment

1. All equipment is operated in a safe manner and the only equipment utilized that which previous training of use has occurred.

2. Defective equipment is reported to the Charge/Staff Nurse.

3. Vacant rooms are neat in appearance.

4. The nurse%u2019s station is maintained.

a. Supplies are maintained at par levels and are arranged in an organized manner.

b. Nurse%u2019s station is neat and orderly.

c. Full waste containers are emptied.

5. Equipment is accounted for on rounds.

a. Ice containers.

b. Geri-chairs.

c. Overbed tables.

d. IV poles (if applicable)

e. Stretcher lift (is applicable)

6. Cleaning and maintenance occurs according to schedule.

a. IV or tube feeding pump poles, tube feeding infusion pumps, and suction machines are wiped down weekly.

b. Utility room is checked routinely for supplies that need to be returned to the appropriate department, meal trays requiring return to dietary, and linen that requires removal to the laundry.

c. .Safety hazards are removed from unit floors (liquid spills, food, paper, equipment cords, etc.)

d. Nourishment refrigerator is cleaned and defrosted once a month.

7. The patient's/resident's environment is safe and clean.

8. Individual patient/resident supplies are re-stocked as indicated.

ADHERENCE TO FACILITY PROCEDURES

1. Procedures are performed according to established method in the procedure manual.

2. Body substance precautions and other appropriate infection control practices are utilized with all activities.

3. Safety guidelines established by the facility (Le., proper restraint and side-rail use) are followed.

4. All accidents or incidents observed are reported on the shift that they occur.

DOCUMENTATION

1. All entries on charts, notes, flow sheets, etc., are recorded in an informative and

descriptive manner.

2. Vital signs are recorded accurately on the appropriate form.

3. Intake and output is accurately recorded on the appropriate form.

4. Patient/resident weights are recorded on the appropriate form.

5. Nursing care flow sheet (if applicable) is maintained.

6. Documentation of restorative program and progress towards goals.

7. Only company approved abbreviations are used when recording information.

COMMUNICATION

1. All changes in a patients/residents condition are reported as soon as possible to the Charge/Staff Nurse.

2. Staff meetings are attended, if on duty. Minutes are read and initiated if not on duty.

3. Patient/resident/unit problems are reported to the Charge Nurse.

PROFESSIONALISM

1. Decisions are made that reflect both knowledge and good judgment.

2. Awareness of own limitation is evident and assistance is sought when necessary.

3. Dress code is adhered to.

4. Committee meetings (if assigned) are attended. Reports related to the committee are given during staff meetings. \

5. Responsibility is taken for own professional growth. Attend and participate in all

scheduled training, educational and orientation classes, programs and activities including annual mandatory classes.

6. Follow work schedules and complete assigned tasks according to the established

policies and procedures of the facility.

7. Notify the facility if you are going to be late or absent from work.

8. Organizational ability and time management is demonstrated.

9. Confidentiality of patient/resident is respected at all times (i.e., when answering telephone and/or speaking to co-workers).

10. Cooperate with inter-departmental personnel to ensure that the needs of the patients/residents are met and maintained.

HUMAN RELATIONS

1. A positive working relationship with patients/residents, visitors and facility staff is

demonstrated.

2. Authority is acknowledged and response to the direction of supervisors is appropriate.

3. Time is spent with patients/residents rather than other personnel.

4. Co-workers are readily assisted as needed.

WORKING CONDITIONS

1. Works in the nursing service areas, patient/resident rooms, nurse%u2019s station, etc.

2. Sits, bends, stands, lifts and moves intermittently throughout the work hours.

3. Subject to exposure to 'infectious waste, diseases, conditions and blood and body fluids.

4. Subject to frequent interruptions.

5. Attends and participate~ in continuing educational programs.

6. Communicates with other nursing and department personnel.

7. Is involved with patients/residents, personnel, visitors, etc., throughout the working hours.
