JOB CLASSIFICATION: Nurse Aide/Orderly

RESPONSIBLE TO: Registered Nurse Supervisor

Director of Nursing

Administrator

QUALIFICATIONS:

1. High school diploma or equivalence is preferred.

2. Should be able to follow oral and written instructions.

3. Must have completed qualified training for nursing assistants and show proof of registration in the state of Iowa.

SKILLS NEEDED:

1. Must have verbal ability to communicate with residents, personnel, visitors, supervisors, peers, and other health personnel.

2. Will spend the majority of on duty hours standing or walking 40% of shift.

3. Will be lifting/pushing/handling resident carts, wheelchairs, equipment, and supplies 50% of shift.

4. Must have visual acuity for accurate reading of thermometers, gauges, etc., and manual dexterity for recording of observations.

5. Must be able to lift seventy-five (75) to one hundred (100) pounds.

JOB SUMMARY:

An aide/orderly is a member of the Health Care Team, supervised by a Registered Nurse or a Licensed Practical Nurse. An aide/orderly assumes the responsibility to demonstrate courtesy, respect, effective relations, and self control toward administrations, supervisors, co-workers, and other employees of the facility. He/she performs direct resident care and related nonprofessional services necessary for the personal needs and comforts of the resident.

JOB DETAIL RESPONSIBILITIES:

1. Demonstrates respect for all residents by observing both the Resident Bill of Rights and Civil Rights Policies.

2. Assist residents with showers, shampoos, bed baths, etc., as designed by the professional nurse.

3. Assist residents with dressing, oral care, combing hair, and other personal hygiene.

4. Changes resident bed lines as scheduled or as indicated.

5. Assist resident at all meals by supervising/prompting food served.

6. Assists residents unable to feed themselves.

7. Serves designated residents snacks or supplementary feedings.

8. Provides residents adequate fluid intake by:

dispensing routine drinking water.

dispensing designated extra fluids and juices.

documents intake when indicated.

reports to nurse in charge of resident's poor intake.

9. Answers residents call lights to determine resident needs.

10. Assists residents with transfer and ambulation, using a gait belt.

11. Transports residents per wheelchair, wheeled reclining chair, cart, or ambulation.

12. Assists residents in self ambulation, using walker, quad cane, wheelchair or other personnel.

13. Assists with treatments/examination as requested by the nurse.

14 Assists resident to vehicles upon discharged transport.

15. Uses mechanical/electric lift, thigh lifts, and gait belts as designated for resident transfer and ambulation.

16. May need to push wheelchair, etc. for residents desiring to be outside a short period of time.

17. Assists RA with physician ordered therapies.

18. Performs designated resident treatments such as:

empty and record amount of urine in foley catheter bags.

perform scheduled perennial and foley catheter care.

turning and positioning residents on a scheduled basis.

obtains vitals as requested, i.e.: temperature, pules, respiration, and weights.

applies elastic bandages, surgical or support hose.

19. Follows resident's Plan of Care as established by physician and the Care Plan Team.

20. Other tasks include:

transfers soiled linen to laundry carts.

provides residents with clean linens on a scheduled or P.R.N. basis.

remakes unit of discharged resident.

cleans soiled utensils, instrument equipment and returns them to appropriate area or central service.

washing and drying of facility/resident laundry.

answers telephone when nurse is unable to come to the telephone.

sorts and transfers clean linen from laundry cart to linen closets.

cleans wheelchairs, closets, and cupboards per established schedule.

21. Responsibilities of the Aide/Orderly:

accepts responsibility of daily bath, BM, and weight flow sheets.

documents resident's daily activities on Resident's ADL form.

performs other treatments or tasks as delegated by charge nurse, RN, or DON.

accepts rotation of scheduled hour of duty as defined in personnel policy.

attends staff inservice monthly meetings in order to meet required twelve (12) hours of inservice per year.

participates in workshops and seminars provided for nursing and facility personnel.

will observe and carry out procedures for safety, infection control, and universal precautions.

knowledgeable of current information on material safety data sheets.

participates in fire, tornado, and disaster drills.

knowledge of CQI program and on-going studies. Will participate in studies as indicated by the CQI Coordinator

performs other duties as deemed necessary by the LPN, RN, or DON.

22. Failure to perform this job to the above specifications could result in undesirable resident consequences (i.e., contracture, pressure sores, decreased mobility, and quality of life) or compromised course of recovery from illness of surgery. Noncompliance may result in termination of employment.
