Description
- Completes and documents initial resident assessments for all admissions and re-admissions and all required ongoing assessments including petitions to the hospital for psych residents.
- Assists new residents and with the orientation to the facility.
- Documentation of any behaviors and incidents of residents during the shift.
- Advocates for the resident during the resident's entire stay with Grasmere Place including admission and discharge.
- Researches and provides information on services to resident members in need of additional support and resources.
- May be asked to participate in Care Plan Conferences.
- Coordinates problem-resolution process across departments that address resident, and or staff concerns.
- Discusses current residents plan of care.
