Social Services Director WOODRUFF CONVALESCENT CENTER (WCC), BELLFLOWER, CA. WCC is looking for a seasoned Social Worker to lead its Social Services department. The Director of Social Services directs the counseling, social services and discharge planning to residents and their families. The primary focus of this position is to assist residents and their families in adjusting from a home or hospital setting to a nursing home environment. This position makes recommendation as to whether a resident can function effectively at home or at other levels within the facility. The candidate assists with discharge planning for homeward bound residents for lower level of placement. To effectively carry out the functions of this position, the candidate must receive and transmit information in a timely manner. The candidate maintains contact with staff from all Departments of the Nursing Home in attempting to remedy the diverse problems of Nursing Home residents.

This position is a full-time position working 40 hours per week.

One year of Long Term Care and Skilled Nursing experience required!
