Job Description

The Director of Social Services reports to the Administrator and ensures that assessments and care plans are developed in a timely and meaningful manner. Provide residents and families with education and support regarding aging, illness, coping and loss. Applicants can demonstrate an understanding of advanced directives, healthcare proxies, guardianship requirements and the like. The Social Work Director acts as the team leader in discharge planning and coordination of services needed for a successful transition back to the appropriate community setting. Qualified candidates must be knowledgeable regarding community resources including VNA, Elder Services, Adult Day Programs, Assisted Living Communities, Rest Homes, Senior Housing, etc. Experience/Requirements Include: Massachusetts Social Worker License (LSW minimally) Bachelor%u2019s Degree from an approved school of Social Work. MSW preferred. -Prior experience working in Social Service department in a Skilled Nursing Facility for 2 years or more required. Great Team, Great Facility! We look forward to hearing from you!
