Eden Home Health is seeking a full time Medical Social Worker to join our team serving Idaho Falls. Our full-time benefits package includes health, vision, and dental insurance, 401K, flexible spending, and vacation time.

Duties Include (but are not limited to):

%u2022 Performs accurate and comprehensive assessments.

%u2022 Obtains physician orders for certification and recertification of home medical social work services.

%u2022 Ensures that changes in patients%u2019 clinical status are communicated to other healthcare team members and HHA office staff verbally and in writing in a timely manner.

%u2022 Ensures that physician orders are communicated to other healthcare team members and HHA staff in a timely, accurate, and thorough manner.

%u2022 Coordinates care planning and delivery with the physician, patient/family, other healthcare team members and HHA staff to facilitate optimal patient outcomes.

%u2022 Educates patient/family and other staff as appropriate, in appropriate medical social work modalities and interventions.

%u2022 Delivers medical social work modalities and interventions appropriate to the patient%u2019s clinical status and needs, and in accordance with the standard of care for patient%u2019s age.

%u2022 Provides, modifies or discontinues medical social work modalities and interventions based upon an ongoing assessment/reassessment of the patient%u2019s clinical status and in accordance with physician orders.

%u2022 Synthesizes assessment data and analyzes data to make clinical judgments regarding patient management.

%u2022 Provides medical social work modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

%u2022 Supports and maintains a culture of safety and quality.

%u2022 Identifies risk of injuries or threats to life and health in the patient%u2019s environment, i.e., environmental/safety/

Education and Experience Requirements:

%u2022 Current valid State clinical social worker license

%u2022 Masters degree from an accredited school of social work, and

%u2022 Two (2) years of post-masters experience in a clinical setting

%u2022 Prior home health experience preferred

%u2022 Valid state driver%u2019s license and reliable automobile

%u2022 Proof of current automobile insurance

Skills:

%u2022 Able to communicate effectively in English, both verbally and in writing.

%u2022 Additional languages preferred.

%u2022 Understands regulations/standards applicable to home healthcare.

%u2022 Knowledge of general medical social work modalities/interventions and scope of medical social work practice within the state of Idaho.
