Description

Eden Home Health is currently looking for dynamic and dedicated Physical Therapists to join our growing team in Arco, ID area.

Essential Functions:

Comply with state practice act and/or American Physical Therapy Association guidelines

Coordinate care and relay information to and with physicians, patient and family caregivers and other healthcare team members in a timely and effective manner

Deliver physical therapy modalities appropriate to patients clinical status and needs

Assess the patients need for durable medical equipment as well as other adaptive therapy

Performs transfer and discharge planning functions in accordance with HHA policy and applicable laws, regulations and standards

Assess patients pain and utilize pain management that includes but is not limited to physical therapy

Provides, modifies or discontinues physical therapy modalities based upon ongoing assessment/reassessment of patients clinical status and in accordance with physicians orders

Participates in interdisciplinary team conferences in accordance with HHA policy

Qualifications:

Must be a graduate of an APTA approved physical therapy program

Must have a current license to practice physical therapy in state of employment

New graduates welcome

Current driver's license and vehicle insurance, and access to a dependable vehicle

Current BCLS certification

Current health certificate/physical examination and TB testing

Prior Home Health Experience preferred but not required

Ability to pass criminal background check
