General Summary:
%u2022 Provides overall planning, direction, coordination, and direct supervision of all nursing and professional services in conjunction with the Director of Patient Care Services.
%u2022 Collaborates with staff to ensure the submission of accurate OASIS data and assists with reconciling errors.
%u2022 Manages the day-to-day operations of the Case Management Team and ensures continuity of patient care.
%u2022 Manages patient and physician phone calls and communications.
%u2022 Provides staff education/training and competency evaluation, including joint home visits.
%u2022 Performs direct patient care as appropriate.
%u2022 Receives referral information and coordinates admissions.
%u2022 Contributes to operational process development.
%u2022 Represents department on committees as requested.
Education and Experience Requirements:
%u2022 Associate's or Bachelor%u2019s degree in Nursing
%u2022 Valid RN license in state of employment
%u2022 Minimum 1 year home health nursing experience preferred
%u2022 Valid driver's license and one year clean driving record
%u2022 Excellent documentation skills
%u2022 Ability to pass a background check
%u2022 Excellent communication skills
%u2022 Performance Improvement
%u2022 EMR experience a plus
%u2022 Home Health experience
