Program Director (LPHA)
Purpose of your Job Position:
The primary purpose of your job position is to direct and oversee the psychiatric program in accordance with current applicable state and local standards, guidelines and regulations, and as directed by the governing board, to assure that the highest degree of quality consumer care is maintained at all times.

%u2022 Provide clinical supervision of QMHPs, MHPs, and RSAs. Group supervision is acceptable, and the size of the group shall be conducive to the provision of clinical supervision. Supervision shall be documented in writing. Supervision of staff shall be for a minimum of one hour per week through face-to face, teleconference or videoconference contact.
%u2022 Sign off on determination of admission requirements
%u2022 Complete consumer initial assessments and annual reassessments
%u2022 Community Access Program
o Review Assessment with IDT
o Ensure physician%u2019s orders are in place
o Ensure care plans are up to date prior to consumer%u2019s first community access
o Restriction of Care plans
%u2022 Manage Discharge Process
o Coordinate Discharge Care Plan Conferences
o Review and sign discharge plans
o Work with community based programming and assist in linkage to programs for continuity of care
%u2022 Therapeutic Home Leave
o Sign off on all consumer requests
o Coordinate with IDT to ensure leaves of absence are therapeutic in nature
%u2022 Assist with investigation interviews as assigned by the Abuse Coordinator
%u2022 Meetings
o Attend morning meeting daily
%uF0A7 Discuss behavioral events and ensure proper follow up has been assigned and completed by an MHP or QMHP
%uF0A7 Discuss impending discharges and any barriers to those discharges
%uF0A7 Discuss any consumer legal matters such as court dates or visits with parole officers
o Treatment Plan meetings
%uF0A7 Review consumer treatment plan and program attendance
%uF0A7 Review of discharge plan
o QAPI
%uF0A7 Responsible for reporting PIPs for departmental concerns
%u2022 Personnel Functions
o Assist in the recruitment, interviewing, and selection of psychiatric rehabilitation staff
o Schedule department work hours (including vacation and holiday schedules), personnel, work assignments, etc.
o Counsel/discipline psychiatric rehabilitation staff as requested for as necessary
o Terminate employees when necessary, documenting and coordinating such actions with the director of human resources and/or Executive Director
%u2022 Perform all other duties as assigned by the Executive Director
