Looking for strong dedicated LVNs to work in fast paced, high acuity unit. Nurses work with RN and DON to support needs of admission, transfers and discharges. IV therapy, wound care, many types of post op procedures and many drains affixed. Rehab treatments work in tandem with nursing care. Great support for peers, team and leadership. Come visit and find a great home. 8 hour shifts scheduled 4 on 2 off. Positions open nights and evenings.

Position: Licensed Vocational Nurses (LVN)

Department: Nursing

Reports To: Director of Nursing

PURPOSE:

The Licensed Practical Nurse-LPN is responsible for managing the residents%u2019 health plan and supervising the day-to-day resident care activities during the shift. Both care management and supervisory responsibilities must be executed in accordance with state and federal regulations and facility policies and procedures.

SUPERVISORY RESPONSIBILITIES:

As required.

ESSENTIAL JOB DUTIES:

Direct day-to-day resident care activities. Inform nursing staff of new admissions, including expected time of arrival, room assignment, etc.

Ensure that daily licensed nursing functions are carried out in accordance with state and federal regulations and community policies and procedures.

Cooperate with the other departments in the community when coordinating resident care services to ensure that residents%u2019 total regimen is maintained.

Complete medical forms, reports, evaluations, studies, charting, etc. as needed.

Ensure that reference materials (PDRs, Regulations, Policies and Procedures, etc.) are maintained and organized at the nurses%u2019 station for quick and easy access by personnel seeking direction.

Admit, transfer, and discharge residents.

Periodically review the residents%u2019 written discharge plan. Participate in updating the written discharge plan as needed.

Review and complete incident/accident reports and conduct investigations as required.

Interpret community policies and procedures to staff, residents, visitors, and government agencies as required.

Make written and oral reports and recommendations to the Director of Nursing Services as needed concerning the operations of the nursing department.

Perform routine charting duties as required and in accordance with facility policies and procedures.

Receive telephone orders from physicians, process them in accordance with facility procedures, and update the care plan and care delivery flow sheets as required.

Record new and changed diet orders and forward the information to the dietary department.

Report all care ordered but not delivered, medication errors, charting errors, diet changes, etc. to the Director of Nursing.

Complete medication and treatment passes in a timely manner and in accordance with state and federal regulation and facility policy.

Attempt to resolve all drug and narcotic discrepancies reported; report all unresolved discrepancies in accordance with state and federal regulation and community policy.

EDUCATION & EXPERIENCE:

Current LPN license in state where employed.

Minimum one year experience as LPN in a nursing facility preferred.

Able to lift a minimum of 25 to 75 pounds.

EXPERIENCE/SKILLS:

Must be able to read, write, and speak fluent English.

Must have excellent communication skills and be able to copy with mental and emotional stress related to position.

Experience working with seniors and/or in the medical field (preferred)

Ability to work in a team environment

Maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment.

Must have excellent observation, good nursing judgment, and good oral and written communication skills.

Professional in attitude, demeanor and dress.

Good organizational skills and ability to work under pressure and meet deadlines.

Ability to be flexible in working hours.
