JOB SUMMARY

Supervise the overall operation of Courtenay Oaks ALF and to assure that resident needs are being met in accordance with regulatory requirements.

RESPONSIBILITIES

Except as specifically noted, the following functions are considered essential to this position:

1. The ability of interview and screen prospective residents to assure appropriate placement within the ALF unit.

2. The ability to provide appropriate follow-up while residents are in the hospital and arrange for continuing care as needed.

3. The ability to establish and supervise health records kept for each resident in accordance with regulatory requirements.

4. The ability to understand regulatory requirements for operation of the ALF and assure compliance.

5. The ability to maintain an effective working relationship with Administrator, ancillary and nursing staff.

6. The ability to interview, hire, evaluate, discipline and discharge employees.

7. The ability to supervise and direct the work activities of those who work on the ALF unit.

8. The ability to develop work schedules and arrange for staff replacement when necessary.

9. The ability to provide for orientation, in-service training and conduct monthly review of fire and evacuation plan.

10. The ability to maintain scheduling flexibility and to share weekend, holiday on-call responsibilities.

11. The ability to arrange for physician appointments, transportation and re-ordering of prescriptions.

12. Responsible for managing the Facility%u2019s inquiry and admission process and provides customers with facility %u2013 related information via facility tours, personal visits/assessments, conversations and follow-up. Coordinates and implements effective facility business development programs. Improves and maintains a positive image of the facility, in addition to maintaining external responsibilities.

Admissions Functions

Responds to inquiry calls from hospital discharge planners, families, and other referral sources. Conducts facility tours. Manages the admission process by maintaining updated bed availability and facility services information at all times. Manages the inquiry process professionally, timely and with appropriate follow-up. Effectively manages occupancy levels and census / premium mix enhancements.

Responsible for completing the required Admission paperwork according to Federal, State and local policy.

Develops and maintains relationships to advise referral sources of bed availability and new products and services, and efficiently meets the daily needs of the community at-large.

Maintains current data base of existing and potential referral sources. Utilizes sales practices and tracking tools.

Alerts appropriate department heads and building staff of projected changes; i.e., admissions, bed changes, and discharges. Utilizes bed management principles. Maintains a close working relationship with the DON and the Business Office Director to assure appropriate and efficient decisions about prospective admissions in accordance with policy. Recommends admission decisions to facility management. When admissions determinations are made, communicates with nursing and housekeeping about appropriate room and bed selection.

Ensures appropriate admission papers and signatures are obtained from residents or responsible parties prior to admission. Communicates special needs of new admits to staff to ensure a smooth transition.

As needed, assists Business Development Consultant with special programs and training initiatives.

Maintains a working knowledge of Federal and state regulations and reimbursement (Managed Care, Medicare and Medicaid). Provides assistance with new associate orientations on these topics and the admission process.

Monitors inquiry/ referral, admissions and census trends. Prepares appropriate reports/tracking logs, and other reports as required. Monitors all active and inactive inquiries. Completes and submits all tracking logs and reports to Administrator and Business Development Consultant in a timely manner.

Maintains a database of medical contacts and community resources, along with knowledge of industry trends and legislative/regulatory issues and communicates this information to facility staff and community contacts.

Monitors and evaluates day to day customer satisfaction. Audiences might include new admissions, post-discharges, current residents and families via in-person visits or phone calls. Shares results with the management team and others as appropriate to develop action plans, as needed.

Develops and implements special events and presentations targeted at community education, establishing and maintaining status for the facility as the expert on skilled nursing, specialty programs, and rehabilitation care in the community.

Must be an active participant in facility meetings providing positive suggestion and input.

Adhere to all Federal, State, local, management comp admissions rules, regulations and policies.

Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.

Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.

Must meet the general health requirements set forth by the policies of this facility which include a medical and physical examination.

Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.

Must be able to push, pull, move, and/or lift a minimum of ____ pounds to a minimum height of ______ feet and able to push, pull, move, and/or carry such weight a minimum distance of ______ feet.

May be necessary to assist in the evacuation of residents during emergency situations.

EDUCATION AND EXPERIENCE

Graduate of an approved school of nursing. Must be licensed as a Registered or Practical Nurse in the State of Florida, and have at least two years experience. Must successfully complete ALF CORE Training Program and pass certification testing, maintain active licensure.

Must be in good physical and mental health. Must be able to get along with other staff members. Must be of good moral character. Must be tactful and patient in communicating with residents and employees. Must have initiative, ambition and good common sense. Must be able to take directions. Must maintain correct personal appearance, hygiene and proper attitude. Must have capacity to grasp new theories and principals and implement them.

WORKING CONDITIONS

Works in clean, well-lighted, well-ventilated building. Sits, stands and walks during the day. Makes decisions regarding resident needs. Communicates with various staff, residents, physician and others. Subject to frequent interruptions. May be required to work odd hours, weekends and holidays to ensure that services are delivered to residents.
