TITLE: LICENSED PRACTICAL NURSE /LICENSED VOCATIONAL NURSE

Below are the job duties that are required of every Licensed Practical Nurse (LPN)/Licensed Vocational Nurse

POSITION SUMMARY

The Licensed Practical Nurse or Licensed Vocational nurse as defined by state, is responsible to the Director of Nursing, and is to be clinically competent; delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with patient, significant others, and other health team members while maintaining standards of professional nursing. Limitation of assessment as designated by scope and state.

The LPN has the primary responsibility for providing medical services, counseling, supervision and program plan implementation to clients during scheduled shifts within known scope of practice. Direct client care involves the following duties:

POSITION RESPONSIBILITIES / STANDARDS

%u2022 Utilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the patient's care, as evidenced by client/staff observation.

%u2022 Assess the patient; collect and document the patient's current health status and medical history. Complete and/or review Admission Data Record as defined by policy. Make accurate and ongoing assessment of patient's status and respond appropriately.

%u2022 Plan/Implement or inter input; set priorities for nursing action in a logical sequence according to patient's needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards. The Plan of Care is initiated on patient admission based on all aspects of the initial assessment as performed by designee within scope. Plan includes physiological, psychosocial, and environmental factors: patient/significant other education and patient discharge planning.

%u2022 Evaluate; nurse can input assessment and revise plan of care based on new data and patient's condition.

%u2022 Document accurate and ongoing assessment of patient status. Document patient care which reflects nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record.

%u2022 Communicate with the physician regarding patient needs, the nursing assessments, and recommendations on a daily basis, and as needed, as evidenced by client feedback and Operations Coordinator observation.

Collaborate with other nurses and health team members to ensure the patient's well being as evidenced by staff/client feedback.

%u2022 Teach patient/significant other appropriate health information in a timely manner and share written informational material as evidenced by client feedback and Operations Coordinator observation.

Perform direct patient care, utilizing established procedures, policies, and standards as evidenced by client/staff feedback and Operations Coordinator observation.

%u2022 Administer medications according to policy and procedure.

%u2022 Administer IV's according to policy and procedure if certified and within scope of practice as designated by state

%u2022 Observe and document patient's response to pertinent medications.

%u2022 Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and policy and procedure (done within a timely manner).

%u2022 Demonstrate necessary skills and knowledge to provide care for patients according to policy and procedure.

Provide personal patient care to provide comfort and well-being to the patient, acknowledging physiological and psychological needs.

Give a thorough, concise report at change of shift.

Communicate pertinent information to the appropriate person in a timely manner.

%u2022 Demonstrate accurate and timely documentation according to form guidelines.

%u2022 Accurately transcribe physician orders according to policy and procedure.

%u2022 Maintain a clean, neat, and safe environment for patient and staff according to policy and procedure as evidenced by staff/client feedback and Operations Coordinator observation.

%u2022 Ensure that supplies and equipment necessary for patient care are stored in an organized and efficient manner.

Demonstrate correct and safe technique in the use of equipment according to specific product information.

Follow appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies.

Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.

%u2022 Adhere to Administrative System/Nursing Service policies as evidenced by Director of Nursing

%u2022 Do not incur excessive overtime.

Remain conscientious in regard to personal hygiene and comply with specific division/unit regulations.

%u2022 100% attendance to mandatory skill updates yearly required.

%u2022 Attend a minimum of four staff meetings and read and sign all minutes of meetings not attended.

POSITION SPECIFICATIONS.

EDUCATION:

%u2022 Graduate of an accredited school of Nursing.

%u2022 Valid registration as a Licensed Practical or Vocational Nurse in State where employee is assumed or valid compact licensure for specified state.

%u2022 Current CPR training from the American Red Cross or American Heart Association

EXPERIENCE:

%u2022 Previous experience in patient care setting preferred.

%u2022 Knowledgeable regarding the nursing process and its application, including the imput of assessment, planning, implementation, and evaluation of nursing care.

PHYSICAL DEMANDS/WORKING CONDITIONS:

%u2022 Intermittent physical activity including walking, standing, sitting, lifting and supporting patients.

%u2022 Incumbent will be exposed to virus, disease and infection from patients and specimens in working environment and high risk assumed

%u2022 Incumbent will be required to work at any facility and be responsible for own transportation.

%u2022 Incumbent will be scheduled based on operational need. (rotate shifts, standby, AA, on-call)

%u2022 Incumbent may be required to work extended periods of time at a video display terminal (VDT).

%u2022 Incumbent may experience traumatic situations including psychiatric, dismembered and deceased patients.
