JOB & PERSONNEL SPECIFICATIONS
JOB CLASSIFICATION: Licensed Practical Nurse
RESPONSIBLE TO: Charge Nurse

QUALIFICATIONS:
1. Must be a graduate of an accredited school of Practical Nursing.
2. Must be currently licensed in the state of Iowa and show proof of licensure.
3. Has completed the LPN supervisory course.
4. Must complete orientation to the health care facility prior to assigned floor duty.
5. It is preferred that the LPN participates in yearly certification in Basic Life Support.
SKILLS NEEDED:
1. Shall be able to use nursing supplies and equipment: Blood pressure devices, thermometers, bags, and tubes, etc..
2. Must be knowledgeable of medical and surgical terms, equipment and supplies.
3. Shall be able to communicate with residents, physicians, supervisors, peers, ancillary personnel and other health personnel.
4. Shall perform most nursing duties standing or walking, lifting and transporting residents and their personal effects 75% of the shift.
5. Shall have visual acuity and manual dexterity to document resident care, operate equipment and handle instruments 25% of shift.
6. Shall be able to adjust work schedule to change and stress situations.
7. Must be able to lift approximately fifty (50) to sixty (60) pounds.
JOB SUMMARY:
A Licensed Practical Nurse is a member of the Health Care Team who renders nursing care under the supervision of a Registered Nurse or LPN designee. He/she performs assigned nursing procedures for the comfort and well being of residents in the nursing facility following the appropriate orientation period. He/she assumes the responsibility to demonstrate courtesy, respect, effective relations, and self-control toward administration, supervisors, co-workers, and other CMHC employees.
JOB DETAIL RESPONSIBILITIES:
1. Demonstrates respect for all residents by observing both the Resident Bill of Rights and Civil Rights Policies.
2. Provides physical care by assisting residents with showers, shampoos, bed baths, oral hygiene, back rubs, nail trim, etc..
3. Assists residents with dressing, undressing, and other personal needs.
4. Assists residents with dietary needs: feeding or assisting residents with meals and nourishment.
5. Providing and documenting supplementary fluid intake.
6. Serving trays.
7. Administers and charts prescribed oral, IM, and P.R.N. medications utilizing the unit dose system.
8. Assist in maintaining residents Medical Records; noting on Nurses Progress Notes and flow sheets to following: medications and reactions if indicated; intake and outputs when indicated; tests performed; T.P.R., B/P, and weights; unusual incidents and changes in residents condition, and emotional stability of residents.
9. May take oral and written physicians orders. Transcribe, note and notify pharmacy of new or changed orders.
10. Maintains active participation in resident's plan of care to assure progress or maintenance of functional level.
11. Assists in admitting new residents to facility and assesses health requirements using the admission assessment sheet. Informs residents and families of resident rights policy.
12. Performs treatments and procedures as listed, but not limited to:
A. Cares for gastronomy, gavage and other tubes.
B. Catheterizes residents.
C. Irrigates foley catheters and completes tubing care.
D. Changes dressings.
E. Applies warm moist packs, cold compresses and dry heat.
F. Takes residents T.P.R., B/P, and weights as ordered.
G. Administers enemas and suppositories
H. Performs accuchecks.
I. Administers vaginal douches and irrigates colostomys.
13. Performs and assists in the following resident care:
A. Proper bed and chair positioning.
B. Use of accepted safety and supportive devices.
C. Control and prevention of decutitus ulcers.
D. Changing of bed linens and/or pads.
14. Assists in resident ambulation, using U walker, quad cane, wheelchair, transporting resident to vehicle and accompanying resident in outside activities.
15. Knowledge in proper lifting and transferring of residents.
16. Knowledgeable in uses of manual/electric lift and gait belt.
17. Assists residents from bed to chair, wheelchair, geriatric chair and/or vice versa.
18. Assists physician and/or professional nurse with special tests, treatments and examinations.
19. Documents pertinent data in cardex (a communication tool between shifts).
20. Supervises nursing staff assigned to designated area and initiates disciplinary action if necessary.
21. Assists professional nurse with daily work assignments, preparing residents charts for routine physician visits, maintaining daily flow charts and forms/duties assigned by professional nurse.
22. Participates in resident and family education pertaining to resident's plan of care.
23. Provides input for documentation on to the resident's care plan.
24. Rotates tour of duty.
25. Will observe and carry out procedures for safety, infection control, and universal precautions.
26. Participates in fire and disaster drills.
27. Assist RN in initiating I.V. therapy, may hang I.V. solution to established I.V. sites that do not contain medication.
28. May regulate by hand or infusion device the rate of non-medicated I.V. and may discontinue I.V.'s with the direction of an R.N..
29. May set-up and apply oxygen equipment.
30. May insert N.G. tubes and feeding tubes which do no require guide wires for insertion.
31. May perform sterile dressing changes.
32. May remove sutures and staples.
33. May perform oral pharyngeal suction.
34. Assists in Schedule II medication count at change of shift daily.
35. Attends monthly staff and inservice meetings, workshops, and seminars for CEU certification and licensure in the state of Iowa.
36. Will be knowledgeable of CQI program and ongoing studies. Will participate in studies as indicated by the CQI Coordinator.
37. Completes other duties as deemed necessary by the supervisor.
38. Failure to perform this job to the above specifications could result in undesirable resident consequences (i.e., contractures, pressure sores, decreased mobility, and quality of life) or compromised course of recovery from illness of surgery. Non-compliance may result in termination of employment.
39. Abides by personnel policy of facility.
