Brookside Inn, a privately owned, skilled nursing and rehabilitation facility in Castle Rock, is seeking a compassionate and conscientious MDS Coordinator, Registered Nurse to join our team of qualified individuals. Rate of Pay

$72,000-$85,000 annually depending on experience

Responsibilities

- Coordinate and assist with completion and submission of accurate and timely interdisciplinary MDS Assessments, CCA%u2019s, and Care Plans according to CMS RAI Manual Regulations and in accordance with all applicable laws, regulations, and Brookside Rehabilitation and Wellness standards.

- Foster a supportive and compassionate environment to care for patients and their families

- Expand knowledge and capabilities by attending educational workshops, conferences etc.

- Maintain strong working knowledge of the Resident Assessment Instrument (RAI)

- Maintain or obtain RAC-CT credentialing through the American Associated of Nurse Assessment Coordinators

- Report any changes in patient%u2019s condition identified by the MDS Assessment to the Director of Nursing

- Provide education to direct care associates regarding updates or changes to the CMS RAI Manual or Skilled Nursing Facility Regulations that impact documentation

- Ability to assist with set-up and review of the Interdisciplinary Comprehensive Care Plan

- Review Final Validation Reports and attest that all assessments have been completed and accepted into the CMS QIES system prior to billing and notify Business Office when assessments are not ready to bill

- Review CMS Reports to identify assessments completed or submitted late and develop systems and processes to prevent reoccurrence

- Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetings upon request.

- Perform functions of a staff nurse as required.

- Assist in the evacuation of patients when necessary.

- Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.

- As part of the Nursing Leadership Team, this position will also have management and leadership expectations as part of their role in the Community including on-call duties per rotation schedule.

- Current, unencumbered, and active license to practice as a Registered Nurse in the state of Colorado

- Ability to assess residents and relate information regarding resident%u2019s condition.

- Must has knowledge of Medicare/Medicaid and insurance reimbursement procedures, contracts, clinical resource utilization and/or case management.

- Basic computer and typing skills

- Must have good interpersonal communication skills, both verbal and written.

- Ability to communicate and function productively on an interdisciplinary team.

- Must be able to read, write, speak, and understand the English language

- Ability to make independent decisions when circumstances warrant such action

- Knowledgeable of nursing and medical practices and procedures as well as regulations and guidelines that pertain to skilled nursing

- Demonstrate dependable, regular attendance

- Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.

- Ability to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel

- Must perform proficiently in all competency areas including but not limited to: general MDS responsibilities, patient care, patient rights, and safety and infection control

- Must be familiar with MDS Coding

Experience:

- Graduate from an accredited school of nursing

- Registered Nurse, 1 year (Preferred)

- Acute care, 1 year (Preferred)

- EMR systems, 1 year (Preferred)

- MDS experience, 1 year (Preferred)
