Aurora Health Management, the fastest growing Long Term Care organization in Connecticut, is searching for a Part time Social Worker to join our team at Aurora Senior Living of East Hartford. We are a Skilled Nursing Home provider that offers both long term care as well as short and long term rehabilitation. We are a supportive, team oriented organization looking to change healthcare delivery for the better.

We are looking for an experienced part time Social Worker. The position of Social Worker provides planning, assessing, coordinating, and implementation of services to enhance each resident%u2019s social and psychosocial well being and assure that care standards are met and the highest degree or quality resident care is provided at all times. The position must function as a team member, team leader and taking direction from the Social Services Director, while focusing on delivering quality care, the position must also manage the resources within their control and assist others in managing resources.

Bachelors or Masters in Social Work

Psych experience in residential or acute psych

Experience in Nursing Home or Medical Social Work

At least 2 years experience working as a Social Worker.

Would like someone with previous experience working in a Skilled Nursing Home Long Term Care setting.
