Description

Seeking a Part Time Social Worker for Presentation . The primary responsibilities of this Social Worker position will include but not limited to: collaborating with each Centers' Directors of Social Services and the interdisciplinary team to ensure a smooth admission process; support residents and families with ongoing adjustment to the facility(s); provide social work role support to our long term care residents; ensuring that assessments and care plans are developed in a timely and meaningful manner; provide residents and families with education and support regarding aging, illness, coping and loss; demonstrates an understanding of advanced directives, healthcare proxies, guardianship requirements and the like. Social Worker acts as assigned social worker for long term residents. Qualified candidates must be knowledgeable regarding community resources including VNA, Elder Services, Adult Day Programs, Assisted Living Communities, Rest Homes, Senior Housing, etc. Experience/Requirements Include: Massachusetts Social Worker License (LSW minimally) Bachelor%u2019s Degree from an approved school of Social Work. MSW preferred. -Prior experience working in Social Service department in a Skilled Nursing Facility for 2 years or more preferred.
