Description

Aurora Senior Living of New Britain is a 282 bed Skilled Nursing Long Term Care Facility. Aurora is searching for a Full Time MDS Registered Nurse to join our team. We are Skilled Nursing Home provider that offers both short and long term care and rehabilitation. We are a supportive, team oriented organization looking to change healthcare delivery for the better.

Candidates for this position will participate in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). The MDS Coordinator manages the overall process and tracking of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for services provided within the Facility.

The MDS Coordinator is responsible for integrating information from nursing, dietary, social services, restorative, rehabilitation and physician services to ensure appropriate reimbursement. Participation in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes; as required by Federal and State regulations.

Requirements:

Registered Nurse currently licensed by the State of Connecticut

Long Term Care Experience

Proficiency in the RAI/PPS process

MDS and Care Plan Experience

Knowledgeable about State and Federal regulations and guidelines governing long-term care

Strong communication, organizational and time management skills
