Description
1. Evaluates and monitors residents' condition and provides professional nursing care in accordance with care plan and doctor's orders
2. Coordinates resident admission, transfer and / or discharge including completing appropriate forms and required documentation in accordance with government regulations and company policies.
3. Performs assessments functions including identification of changes in the resident's physical or psychosocial condition
4.Consults with resident's physician in providing resident care, treatment, rehabilitation and ensures timely
5. Ensures timely documentation, accurate, and descriptive of resident's condition, nursing care provided and resident's response to care.
