Purpose of Your Job Position

The primary purpose of your job position is to plan, organize, and supervise care of the residents in order to meet the total nursing needs of the facility%u2019s residents and to provide medical treatment as prescribed by the attending physician.

As LPN, you are delegated the responsibility and accountability necessary for carrying out your assigned duties.

Receive report from the previous shift. 3

Check attendance of CNAs scheduled to work that shift. 3

Designate special care to be given by CNAs verbally at the shift report. (i.e. ROM, B & B Retraining, ambulation, change of position, treatments, temperature, use of gait belts, training in self care, any cleaning duties that shift, etc.) 3

Supervise serving of meals in resident rooms and dining rooms being sure all residents are being given assistance, if necessary, and noting any changes in eating habits. 2 %uF0D6

Responsible for setting up, administering, charting, and reordering medications for residents. 2 %uF0D6 %uF0D6

Make meaningful nursing observations on residents%u2019 progress notes at least monthly according to facility schedule. Including descriptive admission narrative and discharge summary, residents response or lack of response to medications and treatments, Anticipated short/long term goals, restorative nursing techniques, activity functions, ADL independence, etc. 3 %uF0D6 %uF0D6

Admit, transfer, and discharge residents according to procedures. 2

Responsible for the supervision and continuity of resident care on assigned unit. 2

Supervises, directs, and instructs CNAs. 3

Informs DON of any pertinent changes or information concerning residents, their families, and/or staff members as those changes occur. 2

Responsible for assisting in maintenance of resident care plans, correlating physician orders with the care plan. 3

Make out diet requisitions and changes according to physician orders. Sends forms to Dietary Department. 3

Make rounds with Doctors; discuss possible modalities of treatments, chemotherapy, and restorative nursing techniques. 3 %uF0D6

When an employee is injured, gives first aid, if necessary, and assist employee in filling out incident report and states whatever first aid and/or recommendations given. 3

Check residents receiving treatments to determine effectiveness of current medication and treatment plan, making adjustments under the guidance of the attending physician. 2

Make out resident accident reports in duplicate according to established procedures. One copy to the DON and one copy to the Administrator, being sure to notify physician and family. Notation made on resident%u2019s clinical record vital signs, condition of resident, response, etc. 3

Notify families and/or responsible party regarding change in condition, accidents, injuries, or medical appointments. 3

Attend in-services and shares knowledge gained with staff. 3

Evaluate nursing staff under your supervision with the assistance of the DON and discusses evaluation with employee. 3

Cooperate fully with the Activity, Nursing Rehabilitation, Dietary, and Social Service departments so that thee areas, along with the nursing department, may function together for the benefit of the resident. 3

Give a meaningful report to the oncoming shift including such pertinent information as new admissions, condition of critically ill patients, elevated temperatures, and any other significant knowledge necessary for continuity of care. 3

Evaluated by the DON / ADON at any time. 3

Check resident medical records frequently to be sure:

%u2022 Doctor has visited resident as frequently as prescribed by law

Contact Doctor and/or pharmacy when necessary. 3

Make frequent rounds to all residents and areas noting the following:

%u2022 Appearance and condition of the resident and their room

%u2022 Evaluation of the resident%u2019s physical, mental, social, psychological, and religious needs/complaints.

%u2022 Any breakdown in procedure is noted, immediate intervention is to follow by explaining procedure and rationale to the party involved.

%u2022 Ensure restorative nursing techniques are being carried out.

%u2022 In the bathing area, evaluate bathing procedures and the condition of the resident%u2019s skin, scalp and nails.

%u2022 Cleanliness of utility rooms, unnecessary supplies, tidiness, and locked cabinets.

%u2022 Personal appearance of unit personnel.

%u2022 Necessary supplies for treatment care, paper supplies, utensils, medical, and nursing supplies are well stocked.

Assist in ensuring nursing staff attends in-services when needed. 3

Resident Rights

Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information. 3

Ensure that the resident%u2019s rights to file a complaint are well established and maintained at all times. 3

Working Conditions

Works in office areas as well as throughout the facility and its premises.

Moves intermittently during working hours.

Is subject to frequent interruptions.

Is involved with residents, family members, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.

Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.

Attends and participates in continuing educational programs.

Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.

Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.

May be subject to the handling of and exposure to hazardous chemicals.

Communicates with the medical staff, nursing personnel, and other department supervisors.

Education

Must have a current Licensed Practical Nurse license in good standing with the State of Wisconsin. Cardiopulmonary resuscitation (CPR) training specific to adults, which include a demonstration of the individual%u2019s ability to perform CPR, and has current certification in CPR.

Must be able to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel.

Must not pose a direct threat to the health or safety of other individuals in the workplace.

Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.

Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.

Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.

May be necessary to assist in the evacuation of residents during emergency situations.
