CHARGE NURSE
BASIC FUNCTION
Responsible and accountable for the effective overall management of a nursing unit and the provision of quality nursing care to all residents.
CHARACTERISTIC DUTIES AND RESPONSIBILITIES
ESSENTIAL FUNCTIONS
1. Practices professional nursing that is consistent with the Department of Nursing philosophy, objectives and standards.
2. Participates in the interdisciplinary care approach to patients/residents and families.
3. Assesses the competence and performance of nursing staff as related to position description, standards of care and facility policies.
4. Assists in maintaining safety and cleanliness 'in the work area
5. Participates in the quality assessment and improvement process and activities.
6. Organizes and monitors human and material resources necessary for meeting the needs of the designated nursing unit.
7. Conducts self in a professional manner in compliance with unit and facility policies.
8. Initiates emergency support measures. (i.e., CPR, protects patients/residents from injury).
9. Provides patient/resident/caregiver education as necessary.
10. Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.
11. Maintains knowledge of necessary documentation requirements.
12. Maintains knowledge of equipment set-up, maintenance and use, i.e. monitors, infusion devices, drain devices, etc
13. Directs, supervises and assists care given by other nursing personnel.
MARGINAL FUNCTIONS
1. Participates in the identification of staff educational needs. Serves as a preceptor for new staff as necessary.
2. Participates in the development of unit objectives. Provides input in the formulation and evaluation of standards of care.
EXPOSURE RISK
The Charge Nurse is at high risk for exposure to blood and body fluids when actively providing nursing care to patients/residents. Performance of administrative duties results in minimal exposure risk.
SUPERVISION RECEIVED
Receives administrative direction from the Director of Nursing, Assistant Director of Nursing,
Clinical Coordinator, Nursing Supervisor or designee.
SUPERVISION EXERCISED
Exercises administrative and functional supervision over nursing personnel on a designated unit.
MINIMUM QUALIFICATIONS
1. Current license to practice profession in this state.
2. Graduation from a basic educational program in professional nursing.
3. A minimum of one (1) year nursing experience in a long-term care, sub acute or acute care setting.
4. Previous Charge Nurse or supervisory experience preferred.
MINIMUM PERFORMANCE STANDARDS
Practices professional nursing that is consistent with the Department of Nursing philosophy objectives and standards. Performance in the following areas is acceptable when:
1. Performs personal, physical and cognitive assessments and identifies deviation from norms for defined population.
2. Makes a nursing diagnosis based on the assessments.
3. Identifies goals for patients/residents that are congruent with other disciplines.
4. Participates in interdisciplinary team meetings where goals are mutually established between all disciplines, patients/resident and/or family.
5. Identifies nursing interventions necessary to accomplish established goals.
6. Carries out nursing interventions competently and within the parameters of the Nurse Practice Act and ones' own ability.
7. Evaluates outcome of interventions and revises goals and intervention as necessary based on evaluation.
8. Documents all components of the nursing process in the patient's/resident's permanent medical record.
9. Performs safely and competently those functions delegated by the physician to nursing through written and verbal physician orders.
10. Documents interventions according to facility policy.
11. Reports changes in resident's condition or deviation from prescribed treatments promptly to the physician and supervisor.
12. Assists in the maintenance of the medical record as a legal document of the patient/resident.
Participates in the interdisciplinary care approach to patients/residents and families.
1. Develops and maintains a collaborative relationship with members of other health disciplines.
2. Attends with other appropriate staff, the Interdisciplinary Team Meeting and
contributes to the identification of problems, establishment of measurable goals and the identification of approaches.
3. Communicates outcome of the Interdisciplinary Team Meeting to nursing staff who are caring for the patients/residents.
4. Includes approaches from care plans into daily assignment activities for nursing staff.
5. This includes restorative nursing approaches and assisting the patients/residents to attend activities.
6. Makes written care plans readily available for perusal by involved staff.
Organizes and monitors human and material resources necessary for meeting
needs of given nursing unit.
1. Develops assignment structure and rotation schedule that combine needs of patients/residents with the capabilities of the staff.
2 Adjusts assignment structure to accommodate changing patient/resident mix and
employee capabilities.
3. Communicates with supervisor assignment/patient/resident mix problems.
4. Utilizes the principles of cost accountability in the operation of the nursing unit.
5. Monitors staff work time and authorizes variation from scheduled work time when necessary.
6. Charges supplies utilized by patient/resident according to facility policy.
7. Determines nursing supplies necessary to provide care to a given patient/resident mix and monitors storage on the unit.
8. Monitors the system for chargeable supplies and services used in patient/resident care.
9. Makes recommendations to the Director of Nursing regarding budget adjustments for staff, supplies or capital budget items.
10. Identifies teaching/learning needs of staff and stimulates and supports staff interest and participation 'in in-service education. Where possible provides opportunities for staff development.
11. Establishes, implements and maintains a continuous mechanism of staff communication. Communicates to supervisor concerns and interests of staff.
12. Utilizes feedback from staff in the evaluation of patient/resident care and unit functioning.
13. Establishes, implements and/or maintains communication between shifts related to patients/residents and unit operations issues~
14. Refers non-routine matters to supervisor or administration representative on call.
Becomes involved in the following as directed by the DON/ Administrator/Nursing Supervisor:
a. Responds to public inquiries
b. Emergency drug supply and purchase
c. Directs emergency responses to power failures, fires, natural disasters or criminal activity
d. Handing visitor emergencies such as accidents and injuries
Assess the competence and performance of nursing staff as related to position description, standards of care, and facility policies.
1. Monitors direct care given by Nursing Assistants to ensure compliance with nursing care plan interventions and facility policies.
2. Provides written performance evaluations of nursing staff annually and reviews evaluation with employees to identify expectations, training and/or supportive approaches as directed by the Nursing Supervisor or DON.
3. Keeps anecdotal behavior records when counseling employees. Addresses behavior with employee, define expected changes and has employee sign written counseling.
4. Gives frequent feedback on observation of performance both positive and negative to employees.
5. Counsels employees according to disciplinary process. Reviews possible suspensions or termination with Director of Nursing to ensure all criteria have been met.
Assists in maintaining safety and cleanliness in the work area.
1. Ensures a safe and sanitary environment by:
a. Observing and reporting unsafe or unsanitary conditions to the Director of Nursing or Administrator.
2. Performs nursing functions in a manner that is consistent with safety standards including:
a. Lifting and similar functions.
b. Hand washing and other preventive cross contamination methods.
c. Body substance precautions.
d. Fire and disaster procedures.
3. Supervises and educates Nursing Assistants in safe procedures and enforcing compliance.
a. Instructing Nursing Assistants in safe procedures and enforcing compliance.
b. Instructing Nursing Assistants and patients/residents in fire and disaster procedures and supervising and evaluating activity during drills.
c. Instructing and supervising Nursing Assistants in infection control procedures, including patient/resident isolation.
d. Investigates, documents, and reports patient/resident, employee and visitor accidents and incidents.
WORKING CONDITIONS
1. Works inside the facility throughout the Nursing Service area, including the medication rooms, nurses stations and the patient/resident rooms.
2. Sits, stands, bends, lifts, reaches, walks and moves intermittently during the working hours.
3. Is subject to frequent interruptions.
4. Is subject to a quiet to moderate noise level due to phones, mechanical alarms and occasional construction work.
5. Is involved with patients/residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
6. Is subject to hostile and emotionally upset patients/residents, family members, personnel and visitors.
7. Communicates with the medical staff, nursing personnel, and other department supervisors.
8. Works beyond normal working hours and in other positions temporarily when necessary.
9. Is subject to hazards in the work area including, bums from equipment, odors, exposure to sharp instruments, falls, chemical cleansers etc., throughout the working hours.
10. Is subject to exposure to 'infectious waste, diseases and other conditions.
11. Maintains a liaison with the patients/residents, their families, support departments, etc., to adequately plan for the patients/resident's needs.
12. May be required to wear a face mask, gown or gloves.
SPECIFIC REQUIREMENTS
1. Must possess a current, unencumbered license to practice as a RN in this state.
2. Must be able to read, write, speak and understand the English language.
3. Must possess the ability to make *independent decisions when necessary.
4. Must be able to relate information concerning a patient/resident condition.
PHYSICAL AND SENSQRY REQUIREMENTS
(With or Without the Aid of Mechanical Devices)
1. Must be able to move intermittently throughout the work day.
2. Must be able to speak the English language in an understandable manner.
3. Must be able to cope with the mental and emotional stress of the position.
4. Must be able to see and hear or use prosthetics that will enable the senses to function adequately to ensure that the requirements of this position can be fully met (i.e. accurately read measurements on patient/resident related equipment such as thermometers, monitors, gauges).
5. Must be able to function independently, have personal integrity, flexibility, and the ability to work effectively with patients/residents, personnel and support agencies.
6. Must be in good general health and demonstrate emotional stability.
7. Must be able to relate and work with the disabled, ill, elderly, emotionally upset, and at times hostile people within the facility. .
8. Must be able to lift, push, pull, and move a minimum of 50 pounds.
9. Must be able to assist with the evacuation of patients/residents.
