Description

We are looking for LVN or RN to join our admissions team. We offer excellent benefits Med/Den/Vis/RX.

1. Completes Nursing Admission Evaluations and initiates the care plan, care directives, meal monitor and ADL flow sheets and other documentation forms as appropriate for the individual resident's plan of care.

2. Notifies each discipline within the Center of resident's arrival, validates that the resident room is prepared for new admission.

3. Obtains and records the resident's weight and height at time of admission.

4. Notifies the receiving physician of resident's arrival to the Center.

5. Verifies orders with receiving physician.

6. Communicates admission orders timely to pharmacy.

7. Completes the Medication Administration Record (MAR) and Treatment Administration Record (TAR) at time of admission and reviews with the unit licensed nurse.

8. Completes the day of admit Initial Skin Evaluation; may be assigned by the DNS to complete the next three weekly assessments and Section II of the Initial Skin Evaluation.

9. Completes the day of admit the Fall Risk Evaluation, Wandering Evaluation, and Smoking Evaluation, and initiates safety interventions needed.

10. Schedules an Interdisciplinary Team (IDT) 72 Hour Plan of Care meeting with the resident, family, and/or responsible party and notifies the team members of the date and time of this meeting.

11. Communicates/reports new patient status to the DNS and Unit Supervisor.

12. Enters the new resident on to the 24 Hour Nursing Report and initiates Alert Charting.

13. Assists Medical Records with completing the Admission Medical Record Audit and coordinates with unit supervisor correction of omissions and/or inaccuracies (24 hour through to 21 day audit), provides DNS and Executive Director (ED) report of findings and when corrections are completed.

14. Coordinates with IDT members transfer and discharge processes and coordinates and/or completes nursing documentation related to process.

15. Assists Medical Records with completing the Discharge Medical Record Audit and coordinates with unit supervisor correction of omissions; provides DNS and ED with report of findings when completed.

16. Interprets existing policies and processes to other nursing personnel and nursing assistants, restorative nursing assistants, residents, families, and physicians.

17. Renders immediate assistance to residents in need.

18. Assists with supervision staff on timely response of call-lights and providing services.

19. Observes Center safety policies and processes.

20. Consistently presents an appropriate, professional appearance per Center dress code.

21. Comes to work as scheduled and consistently demonstrates dependability, punctuality, and a professional demeanor.

22. Assumes accountability for knowing and complying with policies and processes contained in the Employee Handbook.

23. Stays current in theory and practices through continuing education and reading.

24. May be assigned other clinical tasks by DNS related to Continuous Quality Improvement.

25. Reviews transfer orders from hospital, coordinates receipt of State required physician documented informed consent for physical restraints and psychotropic medications are sent to Center prior to admission.

EDUCATION, LICENSURE, AND EXPERIENCE

1. Graduate of an approved LVN OR RN program.

2. Licensed in the State of California

3. One year previous supervisory experience in skilled nursing center or hospital setting.
