Description
1. Provides counseling as requested to residents and their families.
2. Completes depression scales and psychosocial assessments for residents and participates in care plan conferences.
3. Completes intake, admission, and discharge procedures for Nursing Home residents. Manages community and hospital referrals.
4. May maintain statistics on resident admissions and discharges.
5. Supervises Psychosocial groups.
6. Manages requests for in-house moves.
7. Resolves roommate difficulties.
